Beteiligung des promigratorischen Moleküls L1CAM an der Epithelialen Mesenchymalen Transition des pankreatischen Gangepithels

Das Pankreasadenokarzinom (PDAC) stellt mit einer 5-Jahres-Überlebensrate von unter 2 % noch immer einen schlecht therapierbaren Tumor dar. Ursächlich sind hierfür sowohl eine Diagnosestellung im fortgeschrittenen Krankheitsstadium mit einhergehender Metastasierung als auch eine ausgeprägte Chemoresistenz zu nennen. Das pankreatische Gangepithel wird als Ursprung für die Tumorigenese gesehen, welche durch Interaktionen mit dem aus vornehmlich Myofibroblasten (PMF) bestehenden Tumorstroma entscheidend gefördert wird. Einer der epidemiologisch gesicherten Risikofaktoren für die Entstehung eines PDACs stellt die chronische Pankreatitis dar, welche wie das PDAC durch eine ausgeprägte Desmoplasie charakterisiert ist. Das Adhäsionsmolekül L1CAM wurde in verschiedenen Tumorentitäten, so auch im PDAC detektiert. Seine vermehrte Expression konnte in unterschiedlichen Studien mit einem chemoresistenten Phänotyp der Tumorzellen assoziiert werden. Da die Mechanismen, über die es zur Hochregulation von L1CAM in tumorösem Gewebe kommt, wenig verstanden sind, war es Ziel dieser Arbeit herauszufinden, ob PMFs bereits in pankreatischen Gangzellen zu einer erhöhten Expression des Adhäsionsmoleküls L1CAM führen und ob diese die maligne Transformation der Epithelzellen fördert. Während mittels immunhistochemischer Färbungen in normalem Pankreasgewebe L1CAM nur in neuronalen Strukturen nachgewiesen werden konnte, konnte in chronischen Pankreatitiden L1CAM auch in Gangstrukturen detektiert werden. In einem in vitro Co-Kulturmodell wurden immortalisierte humane pankreatische Gangepithelzellen (HPDE) mit PMFs über mehrere Wochen co-kultiviert. Dabei wurde eine erhöhte, TGF-beta1 abhängige L1CAM Expression in den Epithelzellen nachgewiesen. Ähnliche Ergebnisse wurden durch TGF-beta1 Stimulation der mono-kultivierten HPDE Zellen erzielt. Sowohl die TGF-beta1 abhängige als auch die durch PMF induzierte L1CAM Expression war unabhängig von der Aktivierung von Smad- Proteinen, jedoch abhängig von der Aktivierung der Mitogen aktivierten Proteinkinase (MAPK) c-Jun N-terminalen Kinase (JNK). Über die Aktivierung von JNK kam es zur Induktion des Transkriptionsfaktors Slug, der seinerseits über die Bindung an zwei Bindungsstellen an den L1CAM Promotor zur Induktion der L1CAM Expression führte. Funktionell konnte die erhöhte Expression des Adhäsionsmoleküls L1CAM in co-kultivierten und TGF-beta1 stimulierten HPDE Zellen mit einer erhöhten Chemoresistenz und Migrationsfähigkeit der Zellen assoziiert werden. Einhergehend mit den funktionellen Veränderungen der co-kultivierten bzw. TGF-beta1 stimulierten Zellen wurden erhöhte Expressionen mesenchymaler Markerproteine (Vimentin, N-Cadherin) sowie ein fibroblastoider Phänotyp festgestellt. Diese Ergebnisse deuten darauf hin, dass die Präsenz der PMFs bzw. die Stimulation mit TGF-beta1 in HPDE Zellen zur Epithelialen Mesenchymalen Transition (EMT) führt, was mit einer Hochregulation von L1CAM verbunden ist. Die Ergebnisse der vorgelegten Arbeit untermauern die Theorie, dass PMFs sowie TGF-beta1 bereits in duktalen Epithelzellen zur malignen Transformation entscheidend beitragen. Sie bieten darüber hinaus einen molekularen Mechanismus, in dem L1CAM eine wesentliche Rolle in der frühen und schnellen Tumorprogression des PDACs einnimmt.

The highly malignant pancreatic ductal adenocarcinoma (PDAC) is characterised by an early metastasis and a pronounced chemoresistance. Tumorgenesis of PDAC is supposed to originate from ductal epithelial cells and is crucially driven by pancreatic myofibroblasts (PMFs), the main compartment of fibrosis in chronic pancreatitis and the tumoral stroma in PDAC. In previous studies it was shown that PMFs essentially contribute to chemoresistance in PDAC cells and that the adhesion molecule L1CAM (CD171), which is highly expressed in PDAC, contributes to the chemoresistant phenotype of PDAC cells. Based on these findings the present thesis aimed to investigate the influence of PMFs on malignant transformation of pancreatic ductal epithelial cells as well as the role of L1CAM in this process. In order to elucidate whether L1CAM expression is already induced in ductal epithelium in premalignant stages, immunohistochemical stainings of L1CAM were performed with tissues of chronic pancreatitis. Whereas in ductal structures of normal pancreatic tissues no L1CAM expression could be detected, ductal structures surrounded by dense fibrotic tissue in chronic pancreatitis exhibited distinct L1CAM expression. In line with these findings co-culture with PMFs led to a TGF-beta1 dependent upregulation of L1CAM expression in the immortalised human pancreatic ductal epithelial cell line HPDE being accompanied by an increased expression of mesenchymal marker proteins such as vimentin and N-cadherin, a spindle-like cell morphology as well as a migratory and chemoresistant phenotype. Accordingly, these alterations which are hallmarks of the epithelial-mesenchymal-transition (EMT) could be observed in HPDE cells after stimulation with exogenous TGF-beta1. Induction of L1CAM expression by PMFs or TGF-beta1 was independent of Smad proteins but dependent on the activation of JNK and the JNK-dependent induction of the transcription factor Slug. Thus, knock down of either L1CAM or Slug abrogated the migratory and chemoresistant phenotype of HPDE cells cultured in the presence of PMFs or TGF-beta1. Furthermore, it could be shown that slug induces L1CAM expression by direct binding to the L1CAM promoter thereby activating L1CAM expression. In summary, these data provide new evidence how an activated microenvironment promote the malignant transformation of ductal epithelial cells and offer a mechanistic explanation for the development and rapid tumor progression of PDAC in which the adhesion molecule L1CAM seems to play an essential role.
